Every participant needs these three forms, filled in and signed by a parent or guardian:

  2. Form 2 Medical & Food Allergy Download ↓
  3. Form 3 Photography & Media Consent Download ↓

Please print, fill in and sign each form, then scan it as a PDF (a mobile scanning app is suitable). Upload one PDF per form. The three PDFs for each participant must not exceed 2 MB in total. Form 2 contains health information and will be accessible only to the Medical and Registration Leads.
